A payroll report with wages by employee for the policy period. |

Names of owners and officers, and a breakdown of their ownership percentages. |

| For Sales‑Based Policies (Property/Liability): |

Reminder of Why You Have to Complete an Audit |
|
| |
| Audits are required for a couple reasons. First, your policy requires us to perform an insurance audit to meet our contractual obligations, and in most states, it’s the law. Secondly, it gives you the opportunity to make sure what you paid for your policy matched the risks you had while it covered you. So, think of it as your policy checkup. Plus, doing the audit can help you avoid a significant fine–up to three times the amount of your original policy premium, depending on your state’s rules. |
