  Thank you for submitting your voluntary payroll report to our Premium Audit Department. We have reviewed the information provided; in order to ensure the most accurate results, please provide additional information for your most recent policy term ending   05/17/2026.

You have reported Non Employee Labor on your voluntary payroll report. In order to support "proof of coverage", Certificates of Insurance are required for those indicated as Non Employee Labor(Precision Metal Manu). We are requesting copies of these items be provided for further review. 

If we are unable to obtain copies of the Certificate of Insurance documents the payroll will be included in our calculations and affect your overall audit premium.

 A response within two (2) business days would be appreciated to help ensure we provide you with your results in a timely fashion.  If we do not receive a response within this timeframe, your audit may result in an increased premium charge.
