This email is to inform you of Magnit Global's screening process to become an approved Consultant, Independent Contractor, or Consulting Organization at Apple Inc. The overall purpose for this screening process is to minimize the reclassification risk to Apple Inc. as well as the Contractor performing the services. The issue of proper worker classification is an area of potential exposure for any business that uses Independent Contractors.
We are requesting the following documents:
* Business Validation Application -Attached
* Articles of Incorporation / Articles of Organization / State Registration documentation (first page only, if applicable)
* Business License from the city in-which TURNKEY PACKAGING SOLUTIONS is located (if applicable)
* W9 -Attached (Please use this updated March 2024 version.)
* Fictitious Business Name Statement (a.k.a. d/b/a, if applicable)
* Business Auto Liability Insurance (or proof of personal auto insurance)
* Workers' Compensation Insurance (or proof of personal health insurance if no W2 employees)
* General Liability Insurance - naming Apple Inc. as an additional insured with the following address:
Apple Inc.
c/o Magnit Global.
2365 Iron Point Road, Suite 270
Folsom, CA 95630
Attn: Facilities
Please note: Due to the time frame we have to work on the screening process, please complete and send screening documents to us as soon as possible in order for us to make a recommendation to Apple Inc. as to your status as an Independent Contractor/Consultant.
