    Hi,

     

    Yes, the manufacturer or shipper is usually responsible for completing the worksheet. Since the items are made of aluminum, the worksheet is required.

    Hi Jemina, 

     

    Would the manufacturer be the one to fill this out? We have ordered these parts before and have never needed multiple customs forms especially stopping the delivery process. These are electronic components and I am not sure what any of these answers would be. They are cameras and lenses i believe.
